Chronic Kidney Disease Management in General Practice handbook 

The third and latest edition of our handbook ‘Chronic Kidney Disease Management in General Practice’, is the synthesis of evolving evidence that shows the importance of early and appropriate management of kidney disease. The Kidney Check Australia Taskforce (KCAT) has produced this handbook to help health professionals identify people who are at risk of kidney disease and manage those who have chronic kidney disease. Kidney Health Check

The Kidney Health Check is a practical approach to detecting chronic kidney disease. It involves three investigations: a blood test to determine the estimated glomerular filtration rate (eGFR), a urine test for albumin creatinine ratio, and a blood pressure check. See details here.

eGFR calculator

The estimated Glomerular Filtration Rate (eGFR) is the best measure of kidney function. An eGFR result is provided when a serum creatinine test is ordered in an adult. The rate can be calculated by using our eGFR calculator. See details here.

‘CKD Go!’ App

CKD Go! is an App to help doctors recognise, understand and manage chronic kidney disease. It allows you to view a personalised action plan, based on the eGFR and urine albumin creatinine ratio. Click here to find out more: CKD Go!

QKidney® risk calculator

This calculator gauges the risk of a person developing moderate to severe kidney disease over the next five years, by assessing their answers to some simple questions. It can be accessed online at no cost by visiting or purchased as an iPhone or iPad app from iTunes here.

Pre-dialysis assessment tools

The Jo-Pre-training Assessment Tool  (JPAT) is a tool used for assessing the suitability of candidates for home dialysis. The JPAT acts as a screening instrument to distinguish suitable candidates for the home dialysis programme, identifying patients with the greatest chance of learning to manage the programme.
